In my case it was a loose ratchet. I can't explain why. Here's the reference from Waller.

First I changed all bushes that were out of tolerance as per Simes chart. The layshaft bushes were at end of life I think, but they are not replaceable by amateurs as far as I can work out? Now resolved. I changed out the gearbox for an unrelated reason as discussed above. To get the primary case to meet the frame lug at the rear, I loosened the engine plates and used a ratchet strap to pivot the engine up and forward a little. The fix seems so obvious now. I rub a bar of soap on the threads, until the thread is full, and use fibre washers. Seems to work alright.
